How to Register a Birth Certificate

Registering a newborn or getting a delayed birth registration. Required for school enrollment, CNIC, passport, and almost everything else.

  • Hospital birth certificate or discharge slip (for newborns)

  • For delayed registration: affidavit and additional witness statements may be required

Register within 60 days (strongly recommended)
Birth registration in Pakistan must be done within 60 days of birth. Registration within this window is free and straightforward. After 60 days it becomes a "delayed registration" which requires more documentation and has a fee.

Get the hospital birth slip
The hospital or clinic where the birth occurred issues a birth slip or certificate. This is your primary document. If born at home, you will need a midwife certificate or a sworn affidavit.

Visit your Union Council (for B-Form)
Go to the Union Council (UC) of your area. Submit the hospital slip, both parents' CNICs, and Nikah Nama. The UC will register the birth in their records and issue a Birth Registration Certificate (BRC).

Visit NADRA for the B-Form
With the UC-issued BRC, visit a NADRA office. NADRA issues the Child Registration Certificate (CRC), commonly called the B-Form. This is the official government document for children under 18.

Delayed registration (born more than 60 days ago)
If more than 60 days have passed, the UC will require additional documentation: a sworn affidavit, witness statements from family members, and possibly a letter from a local council member. Visit the UC first to understand exactly what they need.
  • Register within 60 days — it is free and fast. After that, it becomes much more complicated
  • The B-Form is needed to get the child's passport, enroll in school, and eventually get a CNIC at age 18
  • Keep the original B-Form safe — replacing it requires a full application again
  • Overseas Pakistanis can register their child's birth at a Pakistani Embassy
  • If one parent is a foreign national, additional documentation is required — ask NADRA beforehand
What if my child was born at home and I have no hospital slip?
You will need a sworn affidavit from the parents and ideally a midwife certificate or letter from a local health worker. The Union Council will guide you on the specific documents required for home births.
My child is now 10 years old and was never registered. Is it too late?
No. Delayed registration is possible at any age but requires more paperwork. Visit your Union Council with both parents' CNICs, Nikah Nama, and any existing documents. An affidavit and witness statements will be needed.
Does my child need a B-Form to get a passport?
Yes. The B-Form is required for a child's passport application. Without it, you cannot get a Pakistani passport for your child.
